The Challenge

The healthcare landscape and government policy are shifting to emphasize community-based care over hospital-centered models. This shift requires a significant transformation in service delivery. Health PEI (HPEI) is the single health authority for the Province of Prince Edward Island and is responsible for acting as the sole provider of Home Care services across the island. The Division of Home Care, Palliative Care, and Geriatric Programs within HPEI identified the need to modernize processes and technology to assess and deliver services to clients more effectively.

The Approach

Davis Pier was engaged as an Implementation Partner to design, plan, and implement all aspects of HPEI Home Care’s Service Delivery transformation. Throughout this two-year collaboration, we provided project management, business analysis, privacy, and change management services, helping HPEI with all aspects of selecting and implementing the AlayaCare platform across the province.

Our team has worked with AlayaCare  on several government implementations—putting our expertise in government services and experience in the healthcare sector to work to ensure a seamless, holistic, and successful path to the project delivery.

Alongside a slew of data, our approach included a range of engagements and inputs from key groups that helped inform and shape the solutions—academic partners, service providers, clinical and operational staff, clients, their families, and community.

This work represents a fundamental rethink of assessing homecare or potential homecare clients.

When you’re trying to accurately assess if someone can stay in community, the client assessment is critical to ensure that you are providing them with the right supports. And, providing the team doing these assessments with the right supports is a critical enabler to both change adoption in the healthcare system and how clients receive and experience service.
